By signing this authorization, I approve Perfect Touch Collision Specialists to perform the repairs and/or services requested on the vehicle identified above. This authorization includes the use of parts, materials, and any necessary disassembly required to properly assess and complete the work.

I understand that diagnostic procedures, inspections, and testing may be required to identify vehicle issues, and that such services may result in additional charges. Any repairs or services not specifically listed on this work order will be explained to me and require my approval before completion. If I choose to decline recommended repairs after diagnostics or disassembly have been performed, I understand that applicable diagnostic or teardown charges may still apply and agree to pay those charges.

I authorize Perfect Touch Collision Specialists and its employees to operate, move, and test-drive my vehicle as reasonably necessary for inspection, repair, or quality control purposes.

I acknowledge that Perfect Touch Collision Specialists is not responsible for loss of or damage to personal items left inside the vehicle. Customers are encouraged to remove all valuables before service.

Payment & Texas Legal Notice

I certify that I am the vehicle owner or an authorized agent responsible for payment of all repairs performed. Pursuant to applicable Texas law, I understand that Perfect Touch Collision Specialists may retain possession of the vehicle until payment is made in full.

If payment is made by check, money order, or credit/debit card and is later stopped, declined, or dishonored for any reason, I understand that the vehicle may be subject to repossession or other lawful remedies. I agree to be responsible for all costs associated with collection or repossession, including reasonable attorney’s fees where permitted by law.
